The General Equivalence Mappings (GEM) are maintained by CMS and the CDC to help translate codes between ICD-9-CM and ICD-10-CM classification systems.
GEM files provide forward mappings (ICD-9 to ICD-10) and backward mappings (ICD-10 to ICD-9). These mappings are not always one-to-one due to the expanded specificity in ICD-10.
Note: GEM files were last updated by CMS in 2018. Some newer ICD-10 codes may not have mappings.
The codes have equivalent clinical meaning and can be directly translated.
The mapping is not exact. Clinical review is recommended to ensure accuracy.
Multiple codes may be needed to fully represent the original code's meaning.
No equivalent exists in the target coding system.
- ICD-9 codes start with a digit (0-9), "V" (supplementary factors), or "E" (external causes)
- ICD-10 codes start with a letter A-Z (except U, which is reserved)
- Codes can be entered with or without decimal points (e.g., "250.00" or "25000")
- For approximate mappings, always verify the clinical appropriateness before use
- Some ICD-10 codes added after 2018 may not have corresponding ICD-9 mappings
